Our Care Designer takes the time to understand your needs, goals, and aspirations, involving both you and your family in the discussion. From this, a personalized Care Plan is developed, ensuring the perfect match of caregivers to suit your unique situation.

Our Care Designer from Australian Nurse Care provides ongoing support and care coordination, regularly reviewing your Care Plan and managing your caregivers. Whether it's companionship or nursing care, a few hours a week or 24/7, we work with you to achieve your short-term, medium-term, and long-term goals.

Home Care Nursing

Our Home Care Nursing program is designed to ensure you maintain your personal relationships with family and friends in the comfort of your own home. We collaborate with your medical and allied health team to provide you with the highest level of professional support and care you require. Our experienced care designers monitor changing conditions and assist you in adjusting services as needed.

Our mission is to ensure you can continue to reside in the comfort of your own home while receiving the love and support you need.

Social & Community Participation

We understand the challenges of staying active and engaged, especially for individuals with disabilities. We strive to ensure you feel energized to participate in community-based activities that you enjoy, and if needed, we help develop and maintain friendships.

Around-the-Clock Care

Our comprehensive health services program provides compassionate and attentive assistance whenever you need it, whether it’s 3 in the afternoon or 3 in the early hours of the morning.

From administering medication in the evening to helping you get dressed in the morning, we ensure you have Around-the-Clock Home Care for situations requiring close monitoring. Rest assured, you will always have someone by your side to attend to your needs if necessary.

High Intensity & Complex Health Care

We understand that some individuals may require additional support for complex health needs. Our team of Registered Nurses and specialized staff possess the necessary skills and knowledge to support your high-intensity daily activities, ensuring continuity of safe and high-quality support.

Our qualified and experienced Nurses can provide high-intensity support with complex bowel care, urinary catheters, wound management, Percutaneous Endoscopic Gastrostomy (PEG) feeding, and subcutaneous injections.

Care Design & Coordination

Our Care Designers, all Registered Nurses with years of experience, understand the complexity and individual nature of each person’s disability. They conduct comprehensive assessments and develop care plans utilizing the range of disability services we offer.

Our Care Designers maintain regular ongoing contact to ensure the care team and care plan meet the individual’s needs. This allows for adjustments to be made to improve or enhance care as needed.

We assist you with daily tasks and routines, enabling you to live life on your terms, in your own home. Our mission is to empower maximum independence in personal activities of daily living, such as self-care, household cleaning, and garden maintenance, allowing you to lead your life normally with the confidence that you can accomplish everything you want to with an experienced aged care companion by your side every step of the way.
